Fees and Payment

The encounter form Dr. Marien will provide you at the end of each visit is our guide for the services rendered in the office. The encounter form is also used to schedule tests, consultations and follow-up appointments. The information of the form is critical for appropriate coding and billing. Please give the form to the exit secretary when you leave the office.

Payment is expected from you at the time services are rendered. We accept cash, personal checks, VISA and MasterCard. It is important to us that you have a clear understanding of your responsibility to satisfy your financial obligation for services rendered. If you are experiencing financial difficulties please contact the billing office to discuss payment options. There is a $25 fee for disability and FMLA forms, photographs and peer to peer review for approval of tests or procedures due prior to completion. There will be a $5 monthly service charge for outstanding balances.


We participate with many insurance plans, HMO’s and PPO’s with the exception of Medical Assistance. If you do not have insurance and wish to be seen, please contact the office and an estimate can be provided for the cost of your initial office visit. If we are an “out-of-network” provider for your insurance plan and wish to be seen, please contact our office and we can explain our policy for providing “out-of-network” care.
